I understand that while at Vail Doggie Day Spa my dog will interact with people and other dogs. I am aware that whit the interaction of dogs, there is a chance of injury or even death. As the owner of the above referenced dog(s) I understand that Vail Doggie Day Spa its owners, employees, directors and agents will exercise due care to protect the health and safety of my dog while in their care. I agree to assume all risk of injury to my dog(s) while in the custody of Vail Doggie Day Spa
I give my permission for those in charge to take whatever steps are necessary to obtain medical treatment for my dog(s), and I agree to pay all charges incurred. I consent to the veterinarian of Vail Doggie Day Spa’s choice and in the event of an emergency; I consent to any veterinarian being retained to render care for my dog(s).
In the event my dog causes injury to another dog or to a person while in the custody of Vail Doggie Day Spa I agree to indemnify Vail Doggie Day Spa from any action which might be brought against it, or any defense, settlement, or judgments entered against it. I will assume all liability for the action of my dog(s).
I agree not to file legal charges against Vail Doggie Day Spa, its owners, officers, agents or employees, for any injury, death or loss of my dog without first asserting my claim directly to Vail Doggie Day Spa I hereby waive and release Vail Doggie Day Spa its owners, officers, agents or employees from any and all liability of any nature of injury, death, or loss of my dog resulting from Vail Doggie Day Spa actions or from the actions of my dog or any other dog while in the custody of Vail Doggie Day Spa.
